I, hereby unconditionally, knowingly and voluntarily agree and consent that VIRTUAL STAFFING SOLUTIONS, OPC , referred to as "the Company", may collect, store, access, verify, investigate, use, disclose, process and dispose my personal information or personal data necessary for the purposes of, and in the course of, my job application, employment with the Company, or termination or cessation of such job application, employment relationship, including but not limited to carrying out data analytics, management, profiling, manual or automated decision-making and any activity in accordance with the provisions set forth by Republic Act No. 10173, otherwise known as the Data Privacy Act of 2012.

I further grant my consent to, and hereby authorize the Company to present, share, transmit or transfer my personal information or personal data to third parties to outsource the processing of my personal information to third parties acting as personal information processors on behalf of the Company such as but not limited to background check investigation, school verification, and/or character references, as well as authorize and grant my express, unconditional, voluntary and informed consent to, and hereby authorize, the said Company Affiliates and external service providers, if any, to collect and process my personal information for the above mentioned purposes.

I hereby give and grant my consent to the Company, including its personal information personnel/processors, to share, transmit, disclose and transfer my personal information or personal data to the government, its branches, agencies and/or instrumentalities, for purposes of complying with the Company's mandatory reportorial requirements provided by law and/or regulations, such as but not limited to matters relating to taxation and compliance in statutory requirements.

All my personal information, personal data or any information stated, indicated or contained in my application/General Information Sheet/Resume, including all requirements/documents which I submitted to the Company, I hereby acknowledge that I am solely responsible for the consequences of such disclosure and sharing of said personal information or data. Thus, I hereby hold the Company free and harmless from suits in connection with or arising from privacy breach of my personal data.

I hereby acknowledge and confirm that I have been informed of my rights under the law with respect to my personal information as well as the processing, sharing, or the outsourcing of the collection and processing of my personal information.

Finally, I hereby confirm that I have executed the same of my own volition and free will.

By confirming my agreement below, I represent and warrant that I am the owner of the personal information provided by me and that I have read and understood all of the above provisions and those of the law.
